Medical Release Forms must be notarized and turned into the church secretary during regular church office hours (Mon-Thu 8am-6pm).

In order for adults (18+) to participate in Student and Children’s activities, the top portion of the Name-Based Criminal History Form must be filled in, turned into the church secretary during regular church office house, and resubmitted each year.
